The files are in Excel 2000 format (for Windows). The forms cannot be properly opened with older versions.
1. Input only in colored columns. Noncolored columns are protected against input.
2. The Japanese language input on/off is automatically set.
3. Inputting figures outside the predetermined range (e.g., day, month, and year) will cause an error
indication.
4. Enter a check mark in a check box by clicking the left mouse button near the check box.
5. Characters beyond the maximum number of input characters may not be printed.
Complete and print the forms and submit them to the corresponding office of the school or institute with which the user is affiliated.

with which the user is affiliated. Make sure to submit the three forms,
namely, the Application for Use,
Outline of Research, and Permit for Use, together.
2. Fill in only the fields for extension number and outline in Outline of Research.
3. The content of the fields of Application for Use will be copied into the corresponding fields of Permit for
Use. The user need not fill in the fields of Permit for Use.

Print the appropriate application on A4 paper and submit it to the office of the school or institute with which the user is affiliated.
Print the Application for After-Hours Use on B5 paper, complete the form, sign your name stamp your seal, and submit it following the instructions below.
a)If you submit the form to the Central Institute of Isotope Science in person,
submit the form to the administrative office of the Institute.
b)If you submit the form by e-mail
save the form in PDF format and send it to Info@ric.hokudai.ac.jp as
an attached file.
c)When you submit the form by fax,
fax it to 81-11-706-7862.
※ Complete the necessary information with the signature of the person in charge, and
submit it by a), b), or c). The deadline for submission is 16:00 the
day before the
scheduled day.
Print the Application for Use of Animal Care and Management Facility on
A4 paper, complete the form, sign your name, stamp your seal, and submit
it to the administrative office of the Institute.
Print the Application for Animal Entry on A4 paper, complete the form,
sign your name, stamp your seal, and submit it to the administrative office
of the Institute before the entry of animals.

Label the cage to confirm that the animal in the cage is that on the Application.

Print the Checkup and Inspection Report on A4 paper, complete the form,
sign your name, stamp your seal, and submit it to the administrative office
