【職員健診後の受診勧奨について】
皆様、今年も職員健診を受けられましたか？
労働安全衛生法第66条に基づき、労働者は事業者が行う健康診断を受けなければなりません。
また、健康診断で「要治療」や「要精密検査」の項目があったのに、再受診せず、放置していませんか？
健診後の行動こそが、健康を守るカギです！忙しいからと後回しにしていては、せっかく健診を受けたことが無駄になってしまいます。
保健センターでは、職員健診後の受診勧奨を行っております。
職員健診を「札幌複十字総合健診センター」で受けられた方は、送付された検査結果と一緒に受診勧奨用紙（様式）が入っていますので、病院を受診し、検査の結果をご自身で記載していただき、保健センター（学内便３１）までご送付下さい。
また、ご返事がない方へは、厚生労務室より、再受診勧奨用紙が届きますので、速やかに保健センター（学内便３１）までご返送いただけますよう宜しくお願いいたします。
「人間ドック」を受けられた方については、各健診会場から受診勧奨用紙をお渡しできませんので、こちらからダウンロードしていただき、再受診後に保健センター（学内便３１）までご送付下さい。 ご協力をどうぞよろしくお願いいたします。
If you receive a staff members medical examination at the "Hokkaido Anti-Tuberculosis Association Sapporo Fukujuji Clinic" and there is a detailed examination required as a result, a form is included with the examination result. So please write the result yourself and send it to the Health Care Center (on-campus flight 31). If we do not receive a reply, the Labor and Welfare Division will send you a re-examination form. So please return it to the Health Care Center (on-campus flight 31) immediately. If you receive a staff medical checkup outside of "Hokkaido Anti-Tuberculosis Association Sapporo Fukujuji Clinic" you will not be able to hand over this form. So please download it from here, write the result yourself, and send it to the Health Care Center (on-campus flight 31). Thank you.
